Assessment Form templates

This link from the Royal College website describes the four CBD assessment forms and provides template samples. Our program uses all of these forms. Forms 1, 2 and 4 are located on the Royal College Mainport ePortfolio and MedSIS ePortfolio. Form 3 is found below and must be either printed or emailed for the observer to complete the form. The academic advisor needs to collect completed form 3 and generate a report for EPA 3.2 Managing an inpatient surgical service and for EPA 4.3 Organizing and Managing General OTL-HNS clinics.

Promotion  

The Program Director must meet all residents at least bi-annually to review their performance and progress, discuss career planning and promotion. The academic dossier is a document that summarizes the requirements of the training program. Before meeting the Program Director, residents must complete the following sections (rotations, presentations, teaching sessions, research, publications, temporal bone, courses or workshops attended, extra-curricular undergraduate /postgraduate medical education activities and awards) of their academic dossier. When the academic dossier has been completed and reviewed with the Program Director, the dossier must be signed and posted on the resident’s file.

The Residency Program Committee members and the Program Director must meet to review and discuss all residents’ performance and progress in the spring. The RPC members report back to the residents allowing them enough time to address any concerns that might affect promotion or graduation. A pre-promotion letter is sent to each resident indicating which requirements are either missing or need attention.

The Program Director at the end of the academic year completes a promotion letter.  The promotion letter will indicate whether residents have completed all requirements of the training program or still need to correct any pending conditions.
