This second module covers basic topics in Health and Safety relating to employees within the department. The below safety training is completed through Avenue to Learn website, however you must sign up for training through Mosaic. The link below will give detailed instructions.
All employees must complete the following Enviromental and Occupational Health Support Services (EOHSS) training:
- Fire safety
- Office Workplace Hazardous Materials Information System (WHMIS)
- Slips, Trips & Falls
- Violence in the Workplace
- Fit for N95 mask
- Accessibility for Ontarians with Disabilities Act (AODA)

Upon training completion, please fax test results to Terrie Harvie (Fax#: 905-528-9388) or scan and e-mail to email@example.com.
2.1 Working Alone
It is critical that all employees are aware of Security protocol and Emergency information when working in an office/department area alone. Please download and review one of the following files (according to your site location) with your supervisor regarding the standard operating procedures for administrative staff working alone at any of the Department of Surgery sites.
2.2 Injury/Incident Report
In case of an injury sustained on the job, all Department of Surgery staff must fill out the McMaster Injury/Incident Report form and submit to Terrie Harvie (firstname.lastname@example.org). See the McMaster Injury/Incident Report online form for further details and instructions.
